Abstract
Latar Belakang: Demam berdarah dengue (DBD) masih menjadi masalah kesehatan utama pada anak di negara tropis. Penentuan derajat keparahan secara dini penting untuk mencegah komplikasi. Monocyte-to-Lymphocyte Ratio (MLR) merupakan parameter hematologi sederhana yang mencerminkan respons inflamasi dan berpotensi digunakan sebagai indikator keparahan infeksi dengue. Namun, bukti mengenai peran MLR pada populasi anak masih terbatas.
Tujuan: Mengetahui hubungan antara Monocyte-To-Lymphocyte Ratio (MLR) pada hari keempat demam dengan derajat keparahan infeksi dengue pada pasien anak di RSUD Haji Provinsi Jawa Timur periode Desember 2024 hingga Februari 2025.
Metode: Penelitian ini merupakan studi kuantitatif analitik dengan desain observasional retrospektif potong lintang (cross-sectional) menggunakan data sekunder berupa rekam medis pasien anak <18 tahun dengan diagnosis infeksi dengue. Sampel diambil secara consecutive sampling sesuai kriteria inklusi dan eksklusi hingga diperoleh 48 subjek penelitian. Nilai MLR dihitung dari rasio jumlah monosit terhadap limfosit pada hari ke-4 demam. Derajat keparahan dengue diklasifikasikan berdasarkan kriteria WHO menjadi infeksi dengue ringan dan berat. Analisis hubungan dilakukan menggunakan uji korelasi Spearman.
Hasil: Nilai MLR pada penelitian ini berada pada rentang 0,15– 0,53. Nilai tersebut dihitung berdasarkan jumlah monosit dan limfosit dari pemeriksaan darah rutin dengan satuan sel/µL Median nilai MLR pada kelompok dengue ringan lebih rendah dibandingkan kelompok dengue berat. Dari total 48 pasien, sebanyak 30 pasien (62,5%) termasuk dalam kategori derajat ringan (WHO grade I–II) dan 18 pasien (37,5%) termasuk dalam kategori derajat berat (WHO grade III–IV). Hasil uji korelasi Spearman menunjukkan korelasi positif lemah antara MLR dan derajat keparahan infeksi dengue (r = 0,274), namun hubungan tersebut tidak bermakna secara statistik (p = 0,06).
Kesimpulan: Monocyte-To-Lymphocyte Ratio (MLR) pada hari keempat demam menunjukkan kecenderungan meningkat pada derajat keparahan infeksi dengue yang lebih berat, tetapi hubungan tersebut lemah dan tidak signifikan secara statistik. MLR belum dapat digunakan sebagai indikator tunggal dalam menentukan derajat keparahan infeksi dengue pada pasien anak.

Background: Dengue hemorrhagic fever (DHF) remains a major health problem among children in tropical countries. Early identification of disease severity is crucial to prevent complications. The Monocyte-To-Lymphocyte Ratio (MLR) is a simple hematological parameter that reflects the inflammatory response and has potential as an indicator of dengue severity. However, evidence regarding the role of MLR in the pediatric population is still limited.
Objective: To determine the relationship between the Monocyte-To-Lymphocyte Ratio (MLR) on the fourth day of fever and the severity of dengue infection in pediatric patients at RSUD Haji, East Java Province, during December 2024 until February 2025.
Methods: This study was a quantitative analytic study with a retrospective cross-sectional observational design using secondary data obtained from the medical records of pediatric patients aged <18 years diagnosed with dengue infection. Samples were collected using consecutive sampling based on predefined inclusion and exclusion criteria, resulting in a total of 48 subjects. MLR values were calculated from the ratio of monocyte count to lymphocyte count on the fourth day of fever. Dengue severity was classified according to WHO criteria into mild and severe dengue infection. The correlation between variables was analyzed using Spearman’s rho test.
Results: The MLR values in this study ranged from 0.15 to 0.53, calculated based on monocyte and lymphocyte counts from routine blood examinations (cells/µL). The median MLR value in the mild dengue group was lower than in the severe dengue group. Of the total 48 patients, 30 patients (62.5%) were classified as having mild dengue (WHO grade I–II) and 18 patients (37.5%) as having severe dengue (WHO grade III–IV). Spearman’s rho analysis showed a weak positive correlation between MLR and dengue severity (r = 0.274), but the association was not statistically significant (p = 0.06).
Conclusion: The Monocyte-To-Lymphocyte Ratio (MLR) on the fourth day of fever tended to be higher in more severe dengue infection; however, the correlation was weak and not statistically significant.
