Abstract

Pendahuluan: Bakteremia adalah kondisi serius yang ditandai oleh adanya bakteri di dalam darah, yang dapat meningkatkan risiko morbiditas dan mortalitas pasien, terutama apabila disebabkan oleh bakteri Gram negatif. Neutrophil-Lymphocyte Ratio (NLR) diidentifikasi sebagai biomarker potensial inflamasi sistemik yang mudah diperoleh dan dapat membantu identifikasi pasien berisiko tinggi. Tujuan: Penelitian ini bertujuan untuk mengevaluasi hubungan antara nilai NLR dengan kejadian bakteremia akibat bakteri Gram negatif pada pasien di RSUD Dr. Soetomo Surabaya. Metode: Penelitian ini menggunakan desain observasional analitik dengan pendekatan cross-sectional. Data dikumpulkan dari 457 pasien yang menjalani kultur darah. Analisis bivariat dilakukan menggunakan uji Chi-Square untuk menilai hubungan antara kategori NLR (≥5 dan <5) dengan kejadian bakteremia. Hasil: Sebanyak 315 responden (68,9%) memiliki NLR ≥5, yang secara klinis menunjukkan adanya inflamasi. Hasil uji Chi-Square menunjukkan tidak terdapat hubungan signifikan antara NLR dan kejadian bakteremia akibat bakteri Gram negatif (p = 0,984). Meskipun secara statistik tidak signifikan, Neutrophil-Lymphocyte Ratio (NLR) tetap dapat digunakan sebagai indikator awal inflamasi dan risiko infeksi. Kesimpulan: Meskipun korelasi Neutrophil- Lymphocyte Ratio (NLR) dengan kejadian bakteremia tidak signifikan secara statistik, Neutrophil-Lymphocyte Ratio (NLR) tetap bermanfaat sebagai skrining dini untuk menilai risiko infeksi sistemik. Penggunaan Neutrophil-Lymphocyte Ratio (NLR) harus dikombinasikan dengan evaluasi klinis dan pemeriksaan kultur darah sebagai gold standart.

Introduction: Bacteremia is a serious condition characterized by the presence of bacteria in the bloodstream, which can increase patient morbidity and mortality, particularly when caused by Gram-negative bacteria. The Neutrophil-Lymphocyte Ratio (NLR) has been identified as a potential biomarker of systemic inflammation that is easily obtainable and can assist in identifying high-risk patients. Objective: This study aimed to evaluate the correlation between NLR values and the occurrence of bacteremia caused by Gram-negative bacteria in patients at RSUD Dr. Soetomo Surabaya. Methods: This observational analytic study employed a cross-sectional design. Data were collected from 457 patients who underwent blood culture examinations. Bivariate analysis using the Chi-Square test was performed to assess the relationship between NLR categories (≥5 and <5) and the occurrence of bacteremia. Results: A total of 315 respondents (68.9%) had NLR ≥5, which clinically indicates the presence of inflammation. The Chi-Square test showed no significant correlation between NLR and bacteremia caused by Gram-negative bacteria (p = 0.984). Despite the lack of statistical significance, NLR remains useful as an early indicator of inflammation and infection risk. Conclusion: Although the correlation between NLR and the occurrence of bacteremia was not statistically significant, NLR remains valuable as an early screening tool to assess systemic infection risk. The use of NLR should be combined with clinical evaluation and blood culture testing as the gold standard for diagnosis.
