Abstract

Hipertensi merupakan salah satu penyakit tidak menular dengan prevalensi tinggi di Indonesia dan menjadi faktor risiko utama penyakit kardiovaskular. Penatalaksanaan hipertensi sering memerlukan penggunaan lebih dari satu jenis obat (polifarmasi) untuk mencapai target tekanan darah yang optimal. Kondisi ini meningkatkan risiko Drug Related Problems (DRP) seperti interaksi obat, efek samping, dan rendahnya kepatuhan pasien. Risiko polifarmasi sendiri sangatlah berbahaya bagi kesehatan, interaksi obat-obatan yang terjadi dapat mengurangi efektivitas atau meningkatkan toksisitas terhadap kondisi pasien dan juga menurunkan kepatuhan pasien dalam mengkonsumsi obat. Telefarmasi merupakan bentuk inovasi pelayanan kefarmasian jarak jauh yang memanfaatkan teknologi informasi untuk memberikan edukasi, konseling, dan pemantauan terapi obat kepada pasien. Penelitian ini bertujuan untuk mengkaji peran apoteker dalam layanan telefarmasi terhadap pasien hipertensi dengan resep polifarmasi melalui telaah literatur. Metode yang digunakan adalah literature review dengan pencarian artikel pada basis data Google Scholar, PubMed, dan Science direct dan Taylor&Francis, yang dipilih berdasarkan kriteria inklusi dan eksklusi. Hasil telaah menunjukkan bahwa peran apoteker melalui layanan telefarmasi berkontribusi signifikan dalam meningkatkan kepatuhan minum obat, membantu pengendalian tekanan darah, mengurangi risiko DRP, serta memberikan kenyamanan dan rasa aman bagi pasien. Namun, terdapat kendala yang dihadapi dalam implementasinya, seperti keterbatasan akses internet, rendahnya literasi digital pasien, dan belum adanya regulasi khusus yang mengatur standar layanan telefarmasi di Indonesia. Kesimpulannya, integrasi layanan telefarmasi dengan keterlibatan aktif apoteker berpotensi meningkatkan efektivitas terapi pada pasien hipertensi dengan polifarmasi, terutama jika didukung oleh regulasi, infrastruktur teknologi, dan edukasi berkelanjutan kepada pasien.

Hypertension is one of the most prevalent non-communicable diseases in Indonesia and a major risk factor for cardiovascular disease. The management of hypertension often requires the use of multiple medications (polypharmacy) to achieve optimal blood pressure control. However, polypharmacy increases the risk of drug-related problems (DRPs), including drug–drug interactions, adverse drug reactions, and poor medication adherence. These problems may compromise therapeutic effectiveness, increase the risk of toxicity, and negatively affect patients' adherence to prescribed treatment. Telepharmacy has emerged as an innovative pharmaceutical care service that utilizes information and communication technologies to provide patient education, medication counseling, and remote monitoring of pharmacotherapy. This study aims to examine the role of pharmacists in telepharmacy services for hypertensive patients receiving polypharmacy prescriptions through a literature review. This study employed a literature review methodology by searching articles indexed in Google Scholar, PubMed, ScienceDirect, and Taylor & Francis. Relevant studies were selected based on predetermined inclusion and exclusion criteria. The review findings indicate that pharmacists’ involvement in telepharmacy services contributes significantly to improving medication adherence, supporting blood pressure control, reducing the risk of drug- related problems, and enhancing patients' comfort and confidence in managing their treatment. Nevertheless, several challenges remain, including limited internet accessibility, inadequate digital health literacy among patients, and the absence of specific regulations governing telepharmacy service standards in Indonesia. This study concludes that integrating telepharmacy services with active pharmacist involvement has the potential to improve therapeutic outcomes among hypertensive patients receiving polypharmacy, particularly when supported by comprehensive regulations, adequate technological infrastructure, and continuous patient education.
