Abstract

Pendahuluan: Gangguan perfusi perifer pada penderita diabetes mellitus tipe II akibat hiperglikemia kronis menyebabkan penurunan sirkulasi darah perifer yang ditandai dengan memanjangnya CRT sebagai indikator gangguan perfusi jaringan. Kondisi tersebut dapat meningkatkan risiko terjadinya komplikasi pada ekstremitas bawah. Tujuan: Penelitian bertujuan untuk membandingkan Buerger Allen Exercise (BAE) dan Senam Kaki Diabetik (SKD) terhadap perfusi perifer ekstremitas bawah berdasarkan nilai CRT pada penderita DM tipe II. Metode: Penelitian quasi experimental two-group pretest-posttest dengan teknik purposive sampling pada 50 responden yang dibagi menjadi kelompok BAE (n=25) dan kelompok SKD (n=25) di puskesmas rangkah surabaya. Intervensi dilakukan 3 kali dalam seminggu selama 1 minggu, pengukuran CRT dilakukan sebelum dan sesudah intervensi menggunakan lembar observasi. Analisis data menggunakan uji Wilcoxon Signed Ranks Test dan Mann-Whitney U Test (α=0,05). Hasil dan Pembahasan: berdasarkan uji univariat menunjukkan nilai rata-rata CRT pada kelompok BAE mengalami penurunan dari 2,77 detik saat pre-test menjadi 1,72 detik saat post-test. Sementara itu, pada kelompok SKD, rata-rata nilai CRT juga mengalami penurunan dari 2,88 detik saat pre-test menjadi 1,87 detik saat post-test. Wilcoxon Signed Ranks Test menunjukkan penurunan signifikan nilai CRT pada kedua kelompok setelah intervensi (p= 0,001 < 0,05). Hasil Uji Mann-Whitney menunjukkan adanya kelompok BAE terbukti memberikan dampak penurunan nilai CRT yang lebih besar dan signifikan dibandingkan dengan kelompok SKD. Kesimpulan: Kedua intervensi efektif meningkatkan perfusi perifer, namun BAE lebih efektif dibandingkan SKD dalam mempercepat waktu pengisian kapiler pada penderita DM tipe II. Penelitian ini merekomendasikan BAE sebagai intervensi pilihan utama untuk meningkatkan perfusi perifer, dengan tetap mempertimbangkan kondisi individu pasien.

Introduction: Peripheral perfusion disorders in type II diabetes mellitus patients due to chronic hyperglycaemia led to decreased peripheral blood circulation, characterized by prolonged CRT as an indicator of tissue perfusion disorders. This condition can increase the risk of complications in the lower extremities. Objective: The study aims to compare Buerger Allen Exercise (BAE) and Diabetic Foot Exercise (DFE) on peripheral perfusion of the lower extremities, as measured by CRT values, in type II DM patients. Method: A quasi-experimental two-group pretest-post-test design with purposive sampling technique involving 50 respondents divided into the BAE group (n=25) and the DFE group (n=25) at the Rangkah Surabaya health center. Intervention was conducted 3 times a week for 1 week, and CRT measurements were taken before and after the intervention using an observation sheet. Data analysis was conducted using the Wilcoxon Signed Ranks Test and Mann-Whitney U Test (α=0.05). Results: Based on univariate tests, the average CRT value in the BAE group decreased from 2.77 seconds at pre-test to 1.72 seconds at post-test. Meanwhile, in the SKD group, the average CRT value also decreased from 2.88 seconds at pre-test to 1.87 seconds at post-test. The Wilcoxon Signed Ranks Test showed a significant decrease in CRT values in both groups after the intervention (p = 0.001 < 0.05). The Mann-Whitney Test results show that the BAE group had a greater reduction in CRT values than the SKD group. Conclusion: Both interventions effectively improved peripheral perfusion, but BAE was more effective than SKD in accelerating capillary refill time in type II DM patients. This study recommends BAE as the primary intervention of choice for improving peripheral perfusion, while still considering the individual patient’s condition.
