Abstract

Ketepatan diagnosis laboratorium mengenai hipertrigliseridemia sangat krusial karena menjadi pemicu meningkatnya kasus penyakit kardiovaskular. Namun, akurasinya sering terganggu oleh kesalahan tahap pra-analitik yang mencapai 68%. Salah satunya berupa penundaan pemeriksaan serum tanpa disimpan pada suhu yang tepat dan benar. Hal ini dapat memicu terjadinya hidrolisis analit trigliserida oleh enzim lipase sehingga hasil yang dikeluarkan tidak valid. Penelitian eksperimental semu dengan desain komparatif pretest-posttest ini bertujuan untuk mengetahui perbedaan stabilitas kadar trigliserida serum yang disimpan pada suhu pendingin 4°C dan suhu pembekuan -18°C dalam waktu 7 hari penundaan. Penelitian ini menggunakan 1 subjek yang memenuhi kriteria inklusi dan eksklusi dan direplikasi 9 kali pada setiap kelompok suhu penyimpanan sehingga menghasilkan 27 pengukuran. Hasil penelitian dianalisis secara statistik, dilakukan uji normalitas dan homogenitas yang membuktikan seluruh data penelitian terdistribusi normal (p>0,05) dan sebaran data antar kelompok bersifat homogen (p>0,05) sehingga di uji hipotesis menggunakan uji Paired T-Test yang menunjukkan tidak terdapat perbedaan yang signifikan secara nyata antara penyimpanan suhu 4°C dan -18°C, sebagaimana ditunjukkan oleh nilai p = 0,283 (p>0,05). Namun jika di evaluasi secara teknik laboratorium, penyimpanan pada suhu -18°C lebih dapat mempertahankan stabilitas kadar trigliserida yang didukung dengan data penurunan nilai mean (rata-rata) menjadi 60,07 mg/dL (bias -10,09 mg/dL;persentase -14,38%). Berbeda dengan hasil data penyimpanan suhu 4°C, terjadi penurunan cukup besar yaitu 58,06 mg/dL (bias -12,10 mg/dL;persentase -17,25%). Kesimpulan penelitian ini menunjukkan bahwa secara klinis laboratorium, suhu pembekuan -18°C lebih efektif menjaga stabilitas kadar serum trigliserida dibanding dengan suhu pendingin 4°C.

The accuracy of laboratory diagnosis of hypertriglyceridemia is crucial because it is a trigger for increasing cases of cardiovascular disease. However, its accuracy is often compromised by pre-analytical errors, which account for up to 68%. One such error is delaying serum testing without storing it at the correct temperature. This can trigger hydrolysis of the triglyceride analytes by the lipase enzyme, resulting in invalid results. This quasi-experimental study with a comparative pretest-posttest design aimed to determine the differences in stability of serum triglyceride levels stored at a refrigerated temperature of 4°C and a frozen temperature of -18°C within a 7-day delay. This study used 1 subject who met the inclusion and exclusion criteria and was replicated 9 times in each storage temperature group resulting in 27 measurements. The results of the study were analyzed statistically, normality and homogeneity tests were carried out which proved that all research data were normally distributed (p>0.05) and the distribution of data between groups was homogeneous (p>0.05) so that the hypothesis was tested using the Paired T-Test which showed no significant difference between storage temperatures of 4°C and - 18°C, as indicated by the p value = 0.283 (p>0.05). However, if evaluated by laboratory techniques, storage at -18°C was more able to maintain the stability of triglyceride levels which was supported by data on a decrease in the mean value (average) to 60.07 mg/dL (bias -10.09 mg/dL; percentage -14.38%). In contrast to the results of storage at 4°C, a significant decrease of 58.06 mg/dL (bias -12.10 mg/dL; percentage -17.25%) was observed. The conclusion of this study indicates that, clinically, freezing at -18°C is more effective in maintaining stable serum triglyceride levels than freezing at 4°C.
