Asuhan Keperawatan Nyeri Akut dengan Intervensi Kompres Dingin pada Pasien Postoperasi ORIF Fraktur Humerus Dekstra di Ruang Shofa 3 RSUD Haji Surabaya

Pendahuluan: Nyeri post operasi fraktur dapat mengganggu kenyamanan, mobilisasi, aktivitas, dan proses pemulihan pasien. Pada pasien post operasi Open Reduction and Internal Fixation (ORIF) fraktur humerus dekstra, nyeri dipengaruhi oleh trauma, kerusakan jaringan akibat pembedahan, inflamasi, edema, dan pergerakan ekstremitas. Berdasarkan studi pendahuluan di Ruang Shofa 3 RSUD Haji Surabaya periode Juli–September 2025, dari 45 pasien post operasi fraktur, 37 pasien mengalami nyeri akut (82,2%). Tujuan: Menggambarkan penerapan asuhan keperawatan nyeri akut dengan kompres dingin menggunakan Cold Pack pada pasien post operasi ORIF fraktur humerus dekstra. Metode: Metode deskriptif dengan pendekatan studi kasus pada satu pasien. Pengkajian nyeri menggunakan Visual Analog Scale (VAS). Intervensi Cold Pack diberikan selama 20 menit sebagai terapi nonfarmakologis pendamping selama tiga hari, disertai analgesik sesuai program medis. Hasil: Pasien mengalami nyeri lengan kanan dengan skala VAS 6, berdenyut dan menusuk, bertambah saat ekstremitas digerakkan, serta berkurang saat istirahat. Setelah pemberian Cold Pack selama tiga hari, skala nyeri menurun dari VAS 6 menjadi 5 pada hari pertama, 4 pada hari kedua, dan 3 pada hari ketiga. Kesimpulan: Pemberian kompres dingin menggunakan Cold Pack sebagai terapi pendamping membantu menurunkan intensitas nyeri dan meningkatkan kenyamanan pasien post operasi ORIF fraktur humerus dekstra. Intervensi Cold Pack dapat diterapkan sebagai tindakan keperawatan pendamping untuk membantu mengontrol nyeri akut.

Introduction: Postoperative fracture pain can interfere with patient comfort, mobilization, activities, and the recovery process. In patients following Open Reduction and Internal Fixation (ORIF) surgery for a right humeral fracture, pain is influenced by trauma, tissue damage resulting from surgery, inflammation, edema, and extremity movement. Based on a preliminary study conducted in Shofa 3 Ward at Haji Regional General Hospital Surabaya from July to September 2025, 37 of 45 postoperative fracture patients experienced acute pain (82.2%). Objective: To describe the implementation of nursing care for acute pain using a cold compress with a Cold Pack in a patient following ORIF surgery for a right humeral fracture. Methods: This study employed a descriptive method with a case study approach involving one patient. Pain was assessed using the Visual Analog Scale (VAS). The Cold Pack intervention was administered for 20 minutes as an adjunct non-pharmacological therapy for three days, accompanied by analgesics according to the medical treatment plan. Results: The patient experienced right arm pain with a VAS score of 6, described as throbbing and stabbing, which increased with extremity movement and decreased with rest. After the Cold Pack intervention was administered for three days, the pain score decreased from a VAS score of 6 to 5 on the first day, 4 on the second day, and 3 on the third day. Conclusion: The application of a cold compress using a Cold Pack as an adjunct therapy helped reduce pain intensity and improve comfort in a patient following ORIF surgery for a right humeral fracture. The Cold Pack intervention can be applied as an adjunct nursing intervention to help control acute pain.
