Abstract
Introduction: Tuberkulosis (TB) paru merupakan penyakit infeksi yang disebabkan oleh Mycobacterium tuberculosis dan masih menjadi salah satu masalah kesehatan utama di Indonesia. Pasien TB paru sering mengalami bersihan jalan napas tidak efektif akibat penumpukan sekret yang mengganggu ventilasi dan pertukaran gas. Kondisi tersebut dapat semakin memburuk pada pasien yang menggunakan ventilasi mekanik karena penurunan refleks batuk dan mekanisme pembersihan mukosilier. Oleh karena itu, diperlukan intervensi keperawatan yang tepat untuk mempertahankan patensi jalan napas.
Methods: Penelitian ini menggunakan desain deskriptif dengan pendekatan studi kasus pada satu pasien TB paru yang dirawat di Ruang Intensive Isolasi RSUD Haji Provinsi Jawa Timur selama lima hari. Pengumpulan data dilakukan melalui pengkajian keperawatan, observasi, pemeriksaan fisik, pemantauan status respirasi, dan telaah rekam medis. Intervensi keperawatan yang diberikan meliputi manajemen jalan napas, fisioterapi dada, pemantauan status respirasi, serta tindakan suction sesuai indikasi.
Results: Setelah lima hari pelaksanaan asuhan keperawatan, terjadi penurunan retensi sekret, perbaikan bunyi napas, peningkatan saturasi oksigen, penurunan frekuensi napas, serta pengeluaran sekret yang lebih optimal. Perbaikan tersebut menunjukkan peningkatan efektivitas ventilasi paru dan kondisi respirasi pasien.
Discussion: Penerapan fisioterapi dada dan suction sebagai bagian dari manajemen jalan napas terbukti membantu mobilisasi dan eliminasi sekret pada pasien TB paru dengan bersihan jalan napas tidak efektif. Asuhan keperawatan yang komprehensif, terencana, dan berkesinambungan berkontribusi dalam meningkatkan fungsi respirasi, mempertahankan oksigenasi, serta mencegah komplikasi yang lebih berat, seperti gagal napas.

Introduction: Pulmonary tuberculosis (TB) is an infectious disease caused by Mycobacterium tuberculosis and remains one of the major health problems in Indonesia. Patients with pulmonary TB often experience ineffective airway clearance due to secretion accumulation, which interferes with ventilation and gas exchange. This condition may worsen in patients receiving mechanical ventilation due to decreased cough reflex and impaired mucociliary clearance mechanisms. Therefore, appropriate nursing interventions are required to maintain airway patency.
Methods: This study used a descriptive design with a case study approach involving one pulmonary TB patient who was treated in the Intensive Isolation Room of RSUD Haji East Java Province for five days. Data were collected through nursing assessment, observation, physical examination, respiratory status monitoring, and medical record review. Nursing interventions included airway management, chest physiotherapy, respiratory status monitoring, and suction procedures according to clinical indications.
Results: After five days of nursing care implementation, the patient showed a reduction in secretion retention, improved breath sounds, increased oxygen saturation, decreased respiratory rate, and more optimal secretion clearance. These improvements indicated enhanced pulmonary ventilation effectiveness and respiratory condition.
Discussion: The implementation of chest physiotherapy and suction as part of airway management effectively assisted secretion mobilization and elimination in pulmonary TB patients with ineffective airway clearance. Comprehensive, planned, and continuous nursing care contributes to improving respiratory function, maintaining oxygenation, and preventing more severe complications, such as respiratory failure.
