Abstract

Background: Chronic kidney disease (CKD) is a chronic disease attacking more than 697 million people worldwide. Hypertension is a complication that accelerates the decline in kidney function, making blood pressure control through antihypertensives a crucial aspect of its management. Objective: This study aimed to determine the profile of antihypertensive use in chronic kidney disease patients at the Outpatient Unit of RSUD (regional public hospital) Haji, East Java. Methods: This was a descriptive observational study with retrospective data collection from the medical records of CKD patients from January to December 2025. A total of 80 patients were selected using a purposive sampling technique based on the Slovin formula from a total population of 389 patients. Data were analyzed descriptively and presented in tabular form. Results: The majority of patients were male (56.25%), aged over 65 years (36.25%), and were in stage 5 (75.00%), with the majority undergoing haemodialysis (77.50%). The most common comorbidities were hypertension (100.00%), diabetes mellitus (38.75%), and hypertensive heart disease (32.50%), with 56.25% of patients having controlled blood pressure. Almost all patients experienced an increase in BUN (96.55%), serum creatinine (98.04%), and anaemia (98.44%). The most common antihypertensive group was ARB, with candesartan being the most prevalent drug. Combination therapy was more prevalent than single therapy; four-drug combinations were most common in haemodialysis patients (8.24%) and two-drug combinations in patients without haemodialysis (10.71%). All antihypertensives were administered orally, with once-daily frequency being the most prevalent. Conclusion: Antihypertensive use in CKD patients at RSUD Haji, East Java, aligns with the 2024 KDIGO recommendations, with ARBs as the dominant treatment option.
