Abstract

Rumah sakit merupakan institusi pelayanan kesehatan perorangan yang menyelenggarakan layanan rawat inap, rawat jalan, dan gawat darurat secara paripuma (Permenkes, 2020), meliputi kegiatan promotif, preventif, kuratif, dan rehabilitatif. Seiring perkembangan teknologi, masyarakat semakin kritis dalam menilai mutu pelayanan, sehingga rumah sakit terns berupaya meningkatkan kualitas layanan demi kepuasan pasien.

Keterbatasan penggunaan kode ICD-9-CM dan ICD-10 dalam mengidentifikasi infeksi MDRO terletak pada kerentanannya terhadap kesalahan serta ketidakmampuannya mencerminkan tingkat keparahan komorbid dan kondisi akut secara akurat.

Nilai p-value menunjukkan bahwa tidak terdapat perbedaan yang signifikan secara statistik antara jenis organisme bakteri ESBL dengan distribusi keuangan (p=0,052) maupun jenis kelamin (p=0,291). Namun, terdapat hubungan yang signifikan antara jenis organisme ESBL dengan diagnosis utama pasien (p=0,003). Sementara itu, tidak ditemukan hubungan yang signifikan antara diagnosis utama dengan outcome keuangan berdasarkan tarifBPJS dan biaya rawat inap (p=0,418).

Penelitian ini menunjukkan bahwa pengkodean ICD-10-CM dapat digunakan untuk memperkirakan beban infeksi MDRO di rumah sakit, tetapi perlu diingat bahwa koding tersebut memiliki keterbatasan dan rentan terhadap kesalahan. Hasil penelitian menunjukkan hubungan signifikan antara jenis organisme bakteri ESBL dengan diagnosis utama pasien, yang dapat menentukan komorbiditas dan biaya pasien rawat inap.

A hospital is an individual healthcare institution that provides comprehensive inpatient, outpatient, and emergency care (Ministry of Health Regulation, 2020), encompassing promotive, preventive, curative, and rehabilitative activities. As technology advances, the public becomes increasingly critical in assessing service quality, so hospitals continually strive to improve service quality to ensure patient satisfaction.

The limitations of using ICD-9-CM and ICD-10 codes in identifying MDRO infections lie in their susceptibility to errors and their inability to accurately reflect the severity of comorbidities and acute conditions.

The p-value showed no statistically significant difference between ESBL bacterial type and financial distribution (p=0.052) or gender (p=0.291). However, there was a significant association between ESBL type and the patient's primary diagnosis (p=0.003). Meanwhile, no significant association was found between the primary diagnosis and financial outcomes based on BPJS rates and inpatient costs (p=0.418).

This study demonstrates that ICD-10-CM coding can be used to estimate the burden ofMDRO infections in hospitals, but it is important to note that such coding has limitations and is prone to error. The results show a significant association between ESBL bacterial organism type and patient primary diagnosis, which can influence comorbidities and inpatient costs.
