Abstract

Xerosis cutis merupakan kondisi kulit kering yang sering dijumpai pada populasi geriatri sebagai akibat dari proses penuaan yang memengaruhi struktur dan fungsi kulit. Bertambahnya usia menyebabkan penurunan aktivitas kelenjar sebasea dan kelenjar keringat, berkurangnya kandungan lipid epidermis, menurunnya kadar natural moisturizing factor(NMF), serta meningkatnya transepidermal water loss (TEWL). Perubahan tersebut mengakibatkan berkurangnya kelembapan kulit dan memunculkan berbagai keluhan seperti kulit kering, kasar, bersisik, pruritus, hingga fisura superfisial yang dapat mengganggu aktivitas sehari-hari dan kualitas hidup lansia. Salah satu bahan alami yang berpotensi digunakan sebagai terapi topikal untuk mengatasi kondisi tersebut adalah Virgin Coconut Oil (VCO). Kandungan asam lemak rantai sedang, terutama asam laurat, dalam VCO diketahui memiliki efek emolien, antiinflamasi, dan antimikroba yang dapat membantu menjaga hidrasi kulit serta memperkuat fungsi sawar kulit.
Penelitian ini bertujuan untuk menganalisis efektivitas pemberian Virgin Coconut Oil (VCO) terhadap perbaikan xerosis cutis pada kelompok geriatri di Posyandu Anggrek Rejomulyo Barat Magetan. Penelitian menggunakan pendekatan kuantitatif dengan desain one group pretest-posttest. Pengambilan sampel dilakukan menggunakan teknik purposive sampling berdasarkan kriteria inklusi dan eksklusi yang telah ditetapkan. Sebanyak 36 responden geriatri yang mengalami xerosis cutis dilibatkan dalam penelitian ini. Penilaian tingkat keparahan xerosis dilakukan menggunakan instrumen Overall Dry Skin Score (ODSS) sebelum dan setelah intervensi. Intervensi berupa aplikasi VCO secara topikal pada area kulit yang mengalami xerosis sebanyak dua kali sehari selama periode penelitian. Analisis data dilakukan menggunakan uji Wilcoxon Signed Rank Test untuk mengetahui perbedaan skor xerosis sebelum dan sesudah pemberian VCO.
Hasil penelitian diharapkan menunjukkan bahwa penggunaan VCO mampu meningkatkan kelembapan kulit melalui pemeliharaan integritas lapisan pelindung kulit, mengurangi kehilangan air pada stratum korneum, serta membantu memperbaiki kondisi kulit yang kering dan bersisik. Dengan karakteristiknya yang mudah diperoleh, relatif aman, dan ekonomis, VCO berpotensi menjadi alternatif terapi nonfarmakologis yang efektif dalam penatalaksanaan xerosis cutis pada populasi geriatri.

Xerosis cutis is one of the most common dermatological conditions among the elderly and is closely associated with age-related physiological changes in the skin. Aging leads to decreased activity of sebaceous and sweat glands, reduced epidermal lipid content, diminished levels of natural moisturizing factors (NMF), and increased transepidermal water loss (TEWL). These alterations contribute to impaired skin hydration, resulting in symptoms such as dry, rough, scaly, and itchy skin, as well as superficial fissures that may negatively affect daily activities and quality of life. Virgin Coconut Oil (VCO) has emerged as a potential natural topical treatment due to its high content of medium-chain fatty acids, particularly lauric acid, which possesses emollient, anti-inflammatory, and antimicrobial properties. These characteristics may support skin hydration and enhance the skin barrier function.
This study aimed to evaluate the effectiveness of Virgin Coconut Oil (VCO) in improving xerosis cutis among geriatric individuals at Posyandu Anggrek Rejomulyo Barat, Magetan. A quantitative approach with a one-group pretest-posttest design was employed. Participants were selected using purposive sampling based on predetermined inclusion and exclusion criteria. The study involved 36 elderly individuals diagnosed with xerosis cutis. The severity of xerosis was assessed using the Overall Dry Skin Score (ODSS) before and after the intervention. VCO was applied topically to the affected skin areas twice daily throughout the study period. Data were analyzed using the Wilcoxon Signed Rank Test to determine differences in xerosis severity before and after the intervention.
The use of VCO is expected to improve skin hydration by maintaining the integrity of the skin barrier, reducing water loss from the stratum corneum, and alleviating symptoms associated with xerosis, including dryness, scaling, and roughness. Given its accessibility, safety profile, and affordability, VCO may serve as an effective non-pharmacological alternative for managing xerosis cutis in the geriatric population.
